I think this is a known issue with iOS 9 devices. We're working on solving, but unfortunately I do not have an iOS 9 device at hand so a fix will be delayed.

No, this is for Windows & macOS.

To obtain crash logs, head over to Settings -> Privacy -> Analytics & Improvements -> Analytics Data. Once there, you need to find the crash log pertaining to the app in question which will usually go by the format AppName-Date-Time.ips. Save the file and upload it via DropBox or copy and paste its content.

The only way to make Game Center work is via a paid Apple ID or App+.

Enable the advanced Sideloadly option to support your iOS version.

You’d need Jailbreak to decrypt it or find already decrypted IPAs online. Sideloadly does not work with undecrypted apps, nor does AltStore.

What is your iOS version? Do you have crash logs? Is your IPA decrypted?

You need to disable anti debugger protection (ptrace, syscall). There are tutorials on iOSGods on how to do so. There's also a ptrace pwner deb somewhere. And that LLDB may be outdated.
