To obtain crash logs, head over to Settings -> Privacy -> Analytics & Improvements -> Analytics Data. Once there, you need to find the crash log pertaining to the app in question which will usually go by the format AppName-Date-Time.ips. Save the file and upload it via DropBox or copy and paste its content.

This is the location inside the .IPA. -
I'm sorry about that, I was unable to test it before releasing. I updated the IPA again with Anti-Ban, but I'm unsure if it 100% works so try it on a throwaway account first.

It could be perhaps the bundle ID change, unsure. If you're Jailbroken, try the Normal Install instead of Sideload. Alternatively, you can perhaps use a Cydia tweak that is specifically meant for app duplications.
