Install CrashReporter from Cydia and try to determine what issue is causing the loading loop on the game. Also mark my answer as solved if you can.

Lol. It's pretty obvious this error tells it all, you are missing the prefs header that belongs in /theos/include You shouldn't have problems after that.

Yup same exact problem for me. You will have to boot into safe mode and downgrade substrate, it crashes lower iOS 8 devices.

Hack is already compiled for iOS 9. This issue should not occur, unless you have low RAM.
