General Please Help Me Find This Game

Anyone know what game this could be? Here are the details I remember. I think my IOS was somewhere between IOS 5 to 10.

1. Scary game for IOS, video first person video footage (Real life video footage). I believe there was two games.

2. Choose which way to go by tapping on the side of the screen (Tap on the left side and your played will play the video for that choosing).

3. Start off by running though the woods (Almost all the game is video footages someone recorded in real life).

4. If you click the wrong way you end up dying.

5. You end in the mental (I think) hospital room (Maybe handcuffed to a bed?).

6.  You have to make your way though the hospital by choosing what way to go (Also there are small puzzles you have to solve. Like when trying to open a door you have to enter a code. For example 2212).
