So unfortunately it crashes every time I try to open LDOE. I removed the app, removed the hack, and also removed the keychain data on Filza. It still crashes even after all that and running iCleaner Pro. 

Does this mean I am permanently banned no matter what?

No I had same problem, delet the game from filza try to open game. After than clean using  iCleaner. Now download the game, after that if it's still crash than delete again using filza and open it.The icon will be there after you delete it with fzila
