Nope. Just do what OP said. The "kid-purchased unintentionally" trick works. I really don't know about debit/credit cards because I use iTunes pre-paid gift cards. I'm juggling around $100 USD atm, with no fail at all

Basically you redeem your iTunes gift cards, spend the cash on something on the app store or on in-app purchases, then you file for a refund. You will get your money back via App Store/iTunes account.
