anyway here is a fix for ur problem. 
Go in app store any device doesn't matter which one jailbroken or jailed. Go to Updates>Purchased. Search here for the app.
Or you can use appcake from cydia to get the cracked versions of this app.

The app is deleted from App Store

I can not find anything about the app in question nor can i seem to find an app with the icon you provided.

Checked AppCake for him, nothing there that even looks like the app in question.

Yes, you can copy from other device to PC, then transfer to the new device.
