I still haven't got the soft ban yet so i really don't know what triggers it. Been using this in awakening valley, tower, accidentally use it in arena thrice, runes dungeon, rift. Not tested on guild raids, guild defense, hard tower and story modes. My thoughts is that there is an auto ban on story modes early on that detects high damage so i have been avoiding using it story, take it with a grain of salt.
