nope, this has been occuring ever since my sis gave me her phone, happened both unjailbroken and jailbroken, didnt work in ios8 or ios9. i have no tweaks that would interrupt the volume.

Then I can only guess it's a hardware issue?

 

Why did your sister give you the phone? Maybe she knew it was broken?

This sounds like a hardware issue if it's been happening even after multiple restores.
