Uninstall the Modern Combat 5 from a Cydia repo which you have before installing shmoo's one.

If i format my ipad with itunes then install cydia ???

Yes it wil but don't do that. Just uninstall Modern Combat 5 from Cydia.

How can i unistall modern combat 5 in cydia , i will try but tell me how. Thanks!

It's in your installed packages. Search for it and uninstall it then install the .deb from here.
